There are many. But I am going to say the end of RDR2 and the end of MGS4.

When they killed jonron and el tigre
Yakuza 0. Makoto (girl, blind, rough life) is finally being reunited with her brother. Only he had been tortured and dies right before she gets there. It’s so fucking tragic. Seeing her break down again got to me.

Not really a specific moment, but I had to put down Hellblade: Senua’s Sacrifice because it hit too close to home in regards to the mental issues and started making me cry because of it. Great game, but I could never finish it because of that.
