I always thought that was a odd choice in lyrics.
To be fair they did change it to “your sex is on fire” thus deflecting the STI suspicion elsewhere

“The movement you need is on your shoulder” in Hey Jude was a place holder, but John liked it so Paul kept it. “It’s the best line in the whole song.” John said.
